Request for Letters – Forms – Records (Subject Access Request)

If you would like to request a:

  • Single sheet e.g.: printed results / immunisations history
  • Brief Medical Summary (includes medication list, active problem list, some results)
  • Administrative Letter (Confirmation of registration, University letter)
  • Medical Report (a detailed report written by your GP which incurs a charge determined by the doctor)

then please complete this consent request form and return to Reception with proof of photo ID and proof of address.

You have the legal right to request a copy of the information we hold about you, in line with the General Data Protection Regulation (GDPR).

If you want to see copies of your medical records please use the forms below to submit a subject access request choosing the correct form for which surgery you are registered at.
