New patient registration forms can be downloaded and printed:
GMS1 form + Adult New Patient Registration (Ages 18 and over)
OR
GMS1 form + Child New Patient Registration (Ages under 18)
Accepted Identification
You will be required to provide the practice with two forms of documentation as evidence of identity and one of these must contain a photo in order to complete your registration.
Acceptable documents include passport, photo, driving licences and bank statements, but not utility bills.
